SEN Transition

 

At Pelton Community Primary School, we recognise how important a successful transition is for our pupils with additional needs.

We work in partnership with our feeder secondary schools to provide an enhanced level of transition for those who we feel would benefit from this. This may take the form of additional visits for pupils, one-to-one or small group sessions with secondary school staff, or pupil mentoring from secondary schools.

The period of transition is tailored to the individual needs of each pupil. If appropriate, secondary school colleagues will attend Annual Reviews to meet the current class teacher, parents, and—most importantly—the pupil. This strategy has been particularly effective in easing any worries or concerns that pupils and parents may have. We are committed to involving parents and carers as fully as possible throughout the transition process.

Transition support is also provided within the school, ensuring children moving between year groups are familiar with their new classroom environment and teachers. Designated Safeguarding Leads (DSLs) conduct interviews with parents and carers as part of in-school transfers to ensure that any specific needs, concerns, or support requirements are fully understood and addressed as soon as the child attends our school.

For children starting in our Nursery, the EYFS Lead and Nursery staff visit their previous settings to support a robust and personalised transition. We work closely with health visitors, Early Years Practitioners (EYPs), and families to ensure continuity of care and learning, helping every child settle confidently and achieve the best possible outcomes from the very start.

We ensure that key information travels with the child to support smooth transitions:

  •  CPOMS files and relevant documentation are transferred to receiving schools so all essential information is available to staff.
  • One Page Profiles accompany children, highlighting their strengths, needs, triggers, and strategies that help them thrive. For new starters, One Page Profiles are completed to quickly get staff up to speed and ensure children receive the right support from day one.

New Reception starters are also supported through:

  • Parent Open Information Event
  • Children’s visits to Reception class in the Summer Term
  • Pupils and parents invited for a school lunch

At Pelton Community Primary School, we take a flexible approach, and additional visit opportunities or multi-agency meetings can be arranged if required.
